Due to the changes brought by CFSS, many people may not fully understand how it works and how it differs from PCA. Vista Consultation Service Provider’s goal is to ensure our clients understand what Community First Services and Supports (CFSS) is, how CFSS differs from the PCA program, who is eligible for CFSS, and the services provided under CFSS.

After ensuring that our clients understand what CFSS is and the different models available, Vista Consultation Service Provider works together with them to create an effective plan that supports their needs and outlines the reasons they require CFSS. We provide clients with templates and forms to assist them in this process.

Evaluating and Improving the Quality of Services

Vista Consultation Service Provider evaluates the quality of our work through surveys, allowing us to identify areas for improvement. We also maintain regular dialogues with the individual and their guardian. Changes are made to support plans and approaches as needed, and the feedback we receive from surveys and conversations is used to enhance our quality of service and improve overall life outcomes.

Why Choose Vista Consultation Service Provider?

Vista Consultation Service Provider provides participants and/or their representatives with a notice of action when goods or services are denied. Common reasons for denial include:

  • Services or goods that do not fall within the scope of CFSS

  • Services or goods not related to an assessed need

  • Services or goods included in the plan that do not increase independence or reduce human assistance

Vista Consultation Service Provider coordinates and collaborates, when necessary, with the Care Coordinator or Case Manager when denying or reducing any part of a plan. At the request of the participant, during a change in condition, or in collaboration with the lead agency or DHS, we assist participants in revising their plan by:

  • Not limiting how many times the participant can revise their plan

  • Ensuring the revision is within the scope of CFSS

  • Writing or rewriting the goal(s) associated with the revision, if applicable

  • Re-distributing the revised plan to the agency and/or FMS

  • Providing the information to the Care Coordinator or Case Manager, if applicable

Vista Consultation Service Provider provides participants with a list of all enrolled FMS providers they may choose for their FMS provider. We maintain a policy of neutrality: participants are free to select any enrolled FMS provider without limitation.

Our expertise includes general employment practices, covering:

  • Applicable federal, state, and local tax, labor, and employment laws and regulations

  • Liability and workers’ compensation coverage for household workers

Vista Consultation Service Provider has policies and procedures to guide participants in learning and implementing the skills needed to recruit and select qualified support workers. We assist participants in becoming effective participant-employers by teaching how to:

  • Recruit and select qualified support workers

  • Set wages for support workers

  • Supervise and evaluate support workers

  • Manage the delivery of services by support workers

  • Discharge support workers, if necessary

We consult with and make recommendations to DHS or the Lead Agency regarding involuntary disenrollment from the Budget Model.

Vista Consultation Service Provider is responsible for providing policies and procedures that instruct participants on their responsibilities to monitor their workers. Participant responsibilities include ensuring support workers are competent to meet assessed needs and any additional requirements in the CFSS service delivery plan.

Competency of support workers is determined through evaluation:

  • Within thirty (30) days of a support worker beginning services

  • When there is a change in the participant’s condition

  • When modifications are made to the CFSS service delivery plan

Evidence of support worker competency includes documentation of:

  • Education and experience relevant to assigned responsibilities and participant needs

  • Training received from sources other than the participant

  • Orientation and instruction to implement defined services and supports according to the CFSS service delivery plan

  • Periodic written performance reviews completed at least annually by the participant, based on direct observation of the support worker’s ability to perform job functions

Vista Consultation Service Provider develops a worker training and development plan with or for the participant to ensure support worker competency when:

  • The support worker begins providing services

  • There is any change in the participant’s condition

  • Performance reviews indicate additional training is needed

  • Modifications are made to the CFSS service delivery plan

  • Annual reviews present required documentation related to the participant’s monitoring and evaluation of their workers as outlined above

Vista Consultation Service Provider will:

  • Provide participants with resources on how to find enrolled CFSS agency providers, ensuring participants understand they have full freedom to choose among all enrolled or network CFSS agency providers.

  • Establish policies and procedures that guide participants in learning and implementing the skills needed to recruit and select qualified support workers.

  • Provide participants with a list of Financial Management Service (FMS) providers to facilitate the purchase of goods, making it clear that participants may choose freely from all enrolled FMS providers.

 
 
 

Vista Consultation Service Provider will:

  • Provide timely customer service, responding to participant requests within one (1) business day.

  • Offer a toll-free phone and fax number, TTY support, or alternative methods for communicating with participants who are deaf or hard of hearing.

  • Provide information regarding consultation services on our website.

  • Offer translation and interpreter services, including American Sign Language and support for participants with limited English proficiency.

  • Make materials available to participants in alternate formats.

  • Maintain methods for receiving, returning, and tracking communication from participants and workers.

  • Maintain methods for receiving, responding to, and tracking complaints from participants and workers within a reasonable time frame.

  • Conduct participant satisfaction surveys at least annually, analyze results, and submit summaries to the designated state program agency.

  • Develop and implement a plan of correction or improvement based on survey results to ensure continuous quality improvement of consultation service operations.

  • Develop and implement policies and procedures emphasizing participant direction and cultural competence to communicate effectively with a diverse population of participants of all ages, needs, disabilities, and chronic conditions.

Vista Consultation Service Provider will meet all consultation service goals and objectives:

  • Complete all required contacts for participants who do not have a case manager or care coordinator to authorize CFSS, as well as with workers who are paid parents (for minor participants) or paid spouses.

  • Maintain documentation that each participant’s choice of service delivery model and provider was offered, including:

    • Choice of Agency Model

    • Choice of CFSS agency provider

    • Choice of FMS provider when goods are part of the plan

    • Choice of Budget Model

    • Choice of FMS provider

  • Provide consumer education to participants using self-directed services under a consultant services provider enrolled with the Minnesota Department of Human Services or a PCA agency enrolled with the Minnesota Department of Human Services.

  • Work with participants to develop a community support plan that best meets personal needs and goals.

  • Ensure the personal community support plan may include a mix of paid and unpaid services, and both formal and informal supports, as long as it addresses participant needs and is covered within the Consumer Directed Community Supports (CDCS) budget.
